Aledade Announces Growing Partnerships with Independent Physicians on Fourth Anniversary of Founding
National value-based care network is supporting nearly 300 independent practices, improving care for more than 300,000 patients
Bethesda, Md. – Aledade, Inc., celebrated its fourth anniversary of helping independent physicians move to value-based care by announcing today that its national network has grown to include nearly 300 independent physician practices across 20 Accountable Care Organizations (ACOs) that together are accountable for more than 300,000 lives and $2.5 billion in total health care spending in 30 value-based contracts. Aledade also announced it is forming new ACOs in North Carolina, Georgia, and the greater Philadelphia area — while continuing to expand its presence in existing markets. By expanding its contracting in the Medicare, Medicare Advantage, Medicaid, and commercial insurance markets, Aledade is ensuring that practices can implement improvements across their entire patient panel.
“Aledade was founded on the belief that empowering independent physicians would lead to high-quality, cost-efficient care across all types of communities, patient populations, and payers,” said Farzad Mostashari, MD, founder and CEO of Aledade. “Each year, we’ve proven that Aledade’s physician-led model works and this year, we look forward to partnering with even more physician practices, independent physician associations, Federally Qualified Health Centers (FQHCs), multi-specialty practices, and other physician-led organizations looking to take the next step in value-based care.”
“Aledade is the most successful value-based care initiative operating today. In a short time, Aledade has partnered with hundreds of practices to manage hundreds of thousands of patient lives by offering a totally unique model in this space,” said Bob Kocher, MD, a partner at Venrock, an investor in Aledade and the Chair of its board of directors. “The company’s model has already proven effective by delivering savings and improved health outcomes in both public and private value-based payer arrangements.”
Aledade ACOs provide patients greater access to more informed and more engaged primary care. Over each of the last three ACO program years, practices that joined an Aledade ACO have more than doubled preventive care visits and transitional care services. This has led to a continual decline in unnecessary emergency department visits and readmissions across Aledade ACOs, according to Medicare data. Additionally, patients in Aledade ACOs have seen a 7 percent reduction in total hospitalizations and a 20 percent reduction in skilled nursing facility utilization. For practices, this focus on quality improvement and preventive care has proven rewarding as every practice in all of Aledade’s ACOs earned an exceptional quality bonus payment in 2019 for their performance in the 2017 MACRA MIPS program.
“As an independent practice, joining a physician-led ACO has allowed us to take advantage of new payment arrangements, population health technology and patient data analytics, and a community of other like-minded practices,” said Dr. James Zini of Mountain View, Ark., a member of the Aledade Arkansas ACO. “Aledade helps us to better prepare for and thrive in value-based models that reward our high-quality care and better patient health outcomes.”
At the heart of the Aledade model is the simple, but radical, idea that Aledade only succeeds when partner practices succeed in lowering cost to payers through better care to patients. To further this alignment, the company is taking on two-sided risk (MSSP Track 3) in its ACOs’ second contract periods. Aledade allows independent practices and physician-led ACOs to safely navigate the greater risks and greater rewards of downside risk contracts through expert regulatory understanding, advanced risk analytics, and access to reserve capital.
In addition to Aledade’s business growth, the company marks its fourth birthday by being named one of the Washington Post’s Top Workplaces. In this annual ranking, Aledade was singled out as one of the best companies to work for in the Washington region based on survey results from its employees. The company has grown to 200 employees based at its headquarters in Bethesda, Md. and working throughout the country in its network of ACOs.
