This toolkit is guided by the contents of the Aledade created ‘Stay Well at Home’ checklist, which identifies key areas that must be addressed to help keep vulnerable patients at home.
This guidance is intended to help healthcare providers accurately and completely code and/or bill services that, with proper documentation, may be reimbursable by the Federal Medicare program. This toolkit is not intended to answer every question you might have about 'Stay Well at Home' visits; however, it is the hope of Aledade that it will answer many.
